The pattern appears long before the failure. Complex systems lose contact with reality while continuing to function normally. Function is preserved long after fidelity is lost, and the divergence compounds in silence.
Engineering dissent is present, documented, and overridden. Systems reach deployment with the seeds of failure already built in.
The record exists.
The procedures exist.
The expertise exists.
The failure occurs anyway.
The system has reached a point where reliable contact with reality becomes impossible, yet work continues.
Until recently, no discipline existed to study this pattern directly.
Occasionally someone recognizes the problem from inside it.
They may lack the authority to stop what is happening. They may lack the language to name what they see. But they see what is happening: verification breaking down, signals losing trust, responsibility diffusing, attempts at correction making the system worse.
That recognition is structural perception. It is the foundation this field is built on.
